{"id":11234,"date":"2025-02-08T18:00:00","date_gmt":"2025-02-08T23:00:00","guid":{"rendered":"https:\/\/cov19longhaulfoundation.org\/?p=11234"},"modified":"2025-02-02T23:06:59","modified_gmt":"2025-02-03T04:06:59","slug":"primary-myelofibrosis-jak2-covid-long-haul","status":"publish","type":"post","link":"https:\/\/cov19longhaulfoundation.org\/?p=11234","title":{"rendered":"Primary myelofibrosis, JAK2 &amp; COVID Long-haul"},"content":{"rendered":"\n<p class=\"has-small-font-size\"><a href=\"https:\/\/jitc.bmj.com\/content\/jitc\/10\/4\/e002838.full.pdf\">https:\/\/jitc.bmj.com\/content\/jitc\/10\/4\/e002838.full.pdf<\/a> , JAK inhibitors and COVID-19: Consider Linkage to PMF<\/p>\n\n\n\n<p><strong>Primary myelofibrosis<\/strong>&nbsp;(<strong>PMF<\/strong>) is a rare&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Bone_marrow\">bone marrow<\/a>&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Blood_cancer\">blood cancer<\/a>.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-1\">[1]<\/a><\/sup>&nbsp;It is classified by the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/World_Health_Organization\">World Health Organization<\/a>&nbsp;(WHO) as a type of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Myeloproliferative_neoplasm\">myeloproliferative neoplasm<\/a>, a group of cancers in which there is activation and growth of mutated cells in the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Bone_marrow\">bone marrow<\/a>. This is most often associated with a&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Somatic_mutation\">somatic mutation<\/a>&nbsp;in the&nbsp;<em><a href=\"https:\/\/en.wikipedia.org\/wiki\/JAK2\">JAK2<\/a><\/em>,&nbsp;<em><a href=\"https:\/\/en.wikipedia.org\/wiki\/Calreticulin\">CALR<\/a><\/em>, or&nbsp;<em><a href=\"https:\/\/en.wikipedia.org\/wiki\/Thrombopoietin_receptor\">MPL<\/a><\/em>&nbsp;genes. In PMF, the bony aspects of bone marrow are remodeled in a process called osteosclerosis; in addition, fibroblasts secrete collagen and reticulin proteins that are collectively referred to as&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Fibrosis\">fibrosis<\/a>. These two pathological processes compromise the normal function of bone marrow resulting in decreased production of blood cells such as&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Erythrocyte\">erythrocytes<\/a>&nbsp;(red cells),&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Granulocyte\">granulocytes<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Megakaryocyte\">megakaryocytes<\/a>, the latter cells responsible for the production of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Platelet\">platelets<\/a>.<\/p>\n\n\n\n<p>Signs and symptoms include fever, night sweats, bone pain, fatigue, and abdominal pain. Increased infections, bleeding and an enlarged spleen (<a href=\"https:\/\/en.wikipedia.org\/wiki\/Splenomegaly\">splenomegaly<\/a>) are also hallmarks of the disease. Patients with myelofibrosis have an increased risk of acute meyloid leukemia and frank bone marrow failure.<\/p>\n\n\n\n<p>In 2016,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Prefibrotic_primary_myelofibrosis\">prefibrotic primary myelofibrosis<\/a>&nbsp;was formally classified as a distinct condition that progresses to overt PMF in many patients, the primary diagnostic difference being the grade of fibrosis.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-treatmentalgo-2\">[2]<\/a><\/sup><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"Signs_and_symptoms\">Signs and symptoms<\/h2>\n\n\n\n<p>The primary feature of primary myelofibrosis is bone marrow fibrosis,<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-Tefferi2014-3\">[3]<\/a><\/sup>&nbsp;but it is often accompanied by:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Abdominal fullness related to an&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Splenomegaly\">enlarged spleen<\/a>&nbsp;(splenomegaly).<\/li>\n\n\n\n<li><a href=\"https:\/\/en.wikipedia.org\/wiki\/Hepatosplenomegaly\">Enlargement of both the liver and spleen<\/a>\n<ul class=\"wp-block-list\">\n<li>Splenomegaly due to extramedullary hematopoiesis (hematopoiesis occurring outside of the bone marrow)<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Bone pain<\/li>\n\n\n\n<li>Bruising and easy bleeding due to inadequate numbers of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Platelet\">platelets<\/a><\/li>\n\n\n\n<li>Increased risk of thrombosis<\/li>\n\n\n\n<li><a href=\"https:\/\/en.wikipedia.org\/wiki\/Cachexia\">Cachexia<\/a>&nbsp;(loss of appetite, weight loss, and fatigue)<\/li>\n\n\n\n<li>Fatigue<\/li>\n\n\n\n<li>Fevers<\/li>\n\n\n\n<li>Chills<\/li>\n\n\n\n<li>Weight loss<\/li>\n\n\n\n<li><a href=\"https:\/\/en.wikipedia.org\/wiki\/Gout\">Gout<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hyperuricemia\">high uric acid levels<\/a><\/li>\n\n\n\n<li>Increased susceptibility to infection, such as&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Pneumonia\">pneumonia<\/a><\/li>\n\n\n\n<li>Pallor and shortness of breath due to&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Anemia\">anemia<\/a><\/li>\n\n\n\n<li>Leukoerythroblastic smear (tear-drop RBCs, nucleated RBCs, and immature granulocytes)<\/li>\n\n\n\n<li>In rarer cases, a raised&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Mean_corpuscular_volume\">red blood cell volume<\/a><\/li>\n\n\n\n<li><strong>Cutaneous myelofibrosis<\/strong>&nbsp;is a rare skin condition characterized by&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Dermis\">dermal<\/a>&nbsp;and subcutaneous&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Nodule_(medicine)\">nodules<\/a>.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-Andrews-4\">[4]<\/a><\/sup><sup>:\u200a746\u200a<\/sup><\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"Causes\">Causes<\/h2>\n\n\n\n<p>The underlying cause of PMF is almost always related to an acquired mutation in&nbsp;<em>JAK2<\/em>,&nbsp;<em>CALR<\/em>&nbsp;or&nbsp;<em>MPL<\/em>&nbsp;in a hematopoietic stem\/progenitor cell in the bone marrow.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-5\">[5]<\/a><\/sup>&nbsp;There is an association between&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Mutations\">mutations<\/a>&nbsp;to the&nbsp;<em><a href=\"https:\/\/en.wikipedia.org\/wiki\/JAK2\">JAK2<\/a><\/em>,&nbsp;<em><a href=\"https:\/\/en.wikipedia.org\/wiki\/Calreticulin\">CALR<\/a><\/em>, or&nbsp;<em><a href=\"https:\/\/en.wikipedia.org\/wiki\/Thrombopoietin_receptor\">MPL<\/a><\/em>&nbsp;genes and myelofibrosis.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-TefferiLasho2014-6\">[6]<\/a><\/sup>&nbsp;Approximately 90% of those with myelofibrosis have one of these mutations; 10% do not have mutations in these three genes. These mutations are not specific to myelofibrosis, but are observed in other myeloproliferative neoplasms, specifically&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Polycythemia_vera\">polycythemia vera<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Essential_thrombocythemia\">essential thrombocythemia<\/a>.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-Tefferi2014-3\">[3]<\/a><\/sup><\/p>\n\n\n\n<p>The JAK2 protein is mutated giving risk to a variant protein with an amino acid substitution commonly referred to as V617F; the mutation causing this variant is found in approximately half of individuals with primary myelofibrosis.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-StaerkConstantinescu2014-7\">[7]<\/a><\/sup>&nbsp;The V617F substitution is an amino acid change of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Valine\">valine<\/a>&nbsp;to&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Phenylalanine\">phenylalanine<\/a>&nbsp;at the 617 position in the JAK2 protein. Janus kinases (JAKs) are&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Non-receptor_tyrosine_kinase\">non-receptor tyrosine kinase<\/a>&nbsp;part of the signaling pathway activated by receptors that recognize cytokines and growth factors. These include receptors for&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Erythropoietin\">erythropoietin<\/a>,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Thrombopoietin\">thrombopoietin<\/a>, most&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Interleukin\">interleukins<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Interferon\">interferon<\/a>.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-StaerkConstantinescu2014-7\">[7]<\/a><\/sup>&nbsp;JAK2 mutations play a significant role in the pathogenesis of all the myeloproliferative neoplasms because the recognized mutations all cause constitutive activation of the pathway controlling the production of blood cells arising from&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hematopoietic_stem_cells\">hematopoietic stem cells<\/a>. The V617F subsustition also renders&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hematopoietic_cell\">hematopoietic cells<\/a>&nbsp;more sensitive to growth factors that use JAK2 for&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Signal_transduction\">signal transduction<\/a>, which include&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Erythropoietin\">erythropoietin<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Thrombopoietin\">thrombopoietin<\/a>.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-ThemKralovics2013-8\">[8]<\/a><\/sup><\/p>\n\n\n\n<p>The&nbsp;<em><a href=\"https:\/\/en.wikipedia.org\/wiki\/Thrombopoietin_receptor\">MPL<\/a><\/em>&nbsp;gene codes for a protein that acts as a receptor for thrombopoietin, a growth factor that enhances production of platelets. A mutation in that gene, resulting in the substitution W515L, results in thrombopoietin receptor that is constitutively active even in the absence of thrompoietin. Abnormal&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Megakaryocyte\">megakaryocytes<\/a>&nbsp;redominate in the bone marrow and platelet production is enhanced. The mutant megakaryocytes also release growth factors that stimulate other cells in the bone marrow including fibroblasts, the cells that are stimulated to secrete excess&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Collagen\">collagen<\/a>,<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-Tefferi_2010-9\">[9]<\/a><\/sup>&nbsp;by secreting&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/PDGF\">PDGF<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/TGF-%CE%B21\">TGF-\u03b21<\/a>.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-Schmitz_1995-10\">[10]<\/a><\/sup><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"Mechanism\">Mechanism<\/h2>\n\n\n\n<p>Myelofibrosis is a&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Clone_(cell_biology)\">clonal<\/a>&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Neoplasm\">neoplastic<\/a>&nbsp;disorder of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hematopoiesis\">hematopoiesis<\/a>, the formation of blood cellular components. It is one of the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Myeloproliferative_neoplasm\">myeloproliferative disorders<\/a>, diseases of the bone marrow in which excess cells are produced at some stage. Production of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Cytokine\">cytokines<\/a>&nbsp;such as&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Fibroblast_growth_factor\">fibroblast growth factor<\/a>&nbsp;by the abnormal hematopoietic cell clone (particularly by&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Megakaryocyte\">megakaryocytes<\/a>)<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-11\">[11]<\/a><\/sup>&nbsp;leads to replacement of the hematopoietic tissue of the bone marrow by&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Connective_tissue\">connective tissue<\/a>&nbsp;via collagen&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Fibrosis\">fibrosis<\/a>. The decrease in hematopoietic tissue impairs the patient&#8217;s ability to generate new blood cells, resulting in progressive&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Pancytopenia\">pancytopenia<\/a>, a shortage of all blood cell types. However, the proliferation of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Fibroblast\">fibroblasts<\/a>&nbsp;and deposition of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Collagen\">collagen<\/a>&nbsp;is a secondary phenomenon, and the fibroblasts themselves are not part of the abnormal cell clone.<sup>[<em><a href=\"https:\/\/en.wikipedia.org\/wiki\/Wikipedia:Citation_needed\">citation needed<\/a><\/em>]<\/sup><\/p>\n\n\n\n<p>In primary myelofibrosis, progressive scarring, or&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Fibrosis\">fibrosis<\/a>, of the bone marrow occurs, for the reasons outlined above. The result is&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Extramedullary_hematopoiesis\">extramedullary hematopoiesis<\/a>, i.e. blood cell formation occurring in sites other than the bone marrow, as the hemopoietic cells are forced to migrate to other areas, particularly the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Liver\">liver<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Spleen\">spleen<\/a>. This causes an enlargement of these organs. In the liver, the abnormal size is called&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hepatomegaly\">hepatomegaly<\/a>. Enlargement of the spleen is called&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Splenomegaly\">splenomegaly<\/a>, which also contributes to causing pancytopenia, particularly&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Thrombocytopenia\">thrombocytopenia<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Anemia\">anemia<\/a>. Another complication of extramedullary hematopoiesis is&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Poikilocytosis\">poikilocytosis<\/a>, or the presence of abnormally shaped&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Red_blood_cell\">red blood cells<\/a>.<sup>[<em><a href=\"https:\/\/en.wikipedia.org\/wiki\/Wikipedia:Citation_needed\">citation needed<\/a><\/em>]<\/sup><\/p>\n\n\n\n<p>Myelofibrosis can be a late complication of other myeloproliferative disorders, such as&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Polycythemia_vera\">polycythemia vera<\/a>, and less commonly,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Essential_thrombocythemia\">essential thrombocythemia<\/a>. In these cases, myelofibrosis occurs as a result of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Somatic_evolution_in_cancer\">somatic evolution<\/a>&nbsp;of the abnormal hematopoietic stem cell clone that caused the original disorder. In some cases, the development of myelofibrosis following these disorders may be accelerated by the oral&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Chemotherapy\">chemotherapy<\/a>&nbsp;drug&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Hydroxycarbamide\">hydroxyurea<\/a>.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-Najean_&amp;_Rain-12\">[12]<\/a><\/sup><\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"Sites_of_hematopoiesis\">Sites of hematopoiesis<\/h3>\n\n\n\n<p>The principal site of&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Extramedullary_hematopoiesis\">extramedullary hematopoiesis<\/a>&nbsp;in myelofibrosis is the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Spleen\">spleen<\/a>, which is usually markedly enlarged, sometimes weighing as much as 4000 g. As a result of massive&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Splenomegaly\">enlargement of the spleen<\/a>, multiple subcapsular&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Splenic_infarction\">infarcts<\/a>&nbsp;often occur in the spleen, meaning that due to interrupted oxygen supply to the spleen partial or complete tissue death happens.&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Histologically\">On the cellular level<\/a>, the spleen contains red blood cell precursors,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Granulocyte\">granulocyte<\/a>&nbsp;precursors and&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Megakaryocyte\">megakaryocytes<\/a>, with the megakaryocytes prominent in their number and in their bizarre shapes. Megakaryocytes are believed to be involved in causing the secondary fibrosis seen in this condition, as discussed under &#8220;Mechanism&#8221; above. Sometimes unusual activity of the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Red_blood_cell\">red blood cells<\/a>,&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/White_blood_cell\">white blood cells<\/a>, or&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Platelet\">platelets<\/a>&nbsp;is seen. The liver is often moderately enlarged, with foci of extramedullary hematopoiesis. Microscopically, lymph nodes also contain foci of hematopoiesis, but these are insufficient to cause enlargement.<sup>[<em><a href=\"https:\/\/en.wikipedia.org\/wiki\/Wikipedia:Citation_needed\">citation needed<\/a><\/em>]<\/sup><\/p>\n\n\n\n<p>There are also reports of hematopoiesis taking place in the&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Lungs\">lungs<\/a>. These cases are associated with&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Pulmonary_arterial_hypertension\">hypertension in the pulmonary arteries<\/a>.<sup><a href=\"https:\/\/en.wikipedia.org\/wiki\/Primary_myelofibrosis#cite_note-Trow_et_al-13\">[13]<\/a><\/sup><\/p>\n\n\n\n<p>The&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Bone_marrow\">bone marrow<\/a>&nbsp;in a typical case is hypercellular and diffusely&nbsp;<a href=\"https:\/\/en.wikipedia.org\/wiki\/Fibrotic\">fibrotic<\/a>.
