{"id":15780,"date":"2026-09-03T06:00:00","date_gmt":"2026-09-03T10:00:00","guid":{"rendered":"https:\/\/cov19longhaulfoundation.org\/?p=15780"},"modified":"2026-08-21T08:46:23","modified_gmt":"2026-08-21T12:46:23","slug":"mitochondrial-and-metabolic-dysfunction-in-long-covid-a-bioenergetic-model-of-fatigue-post-exertional-malaise-and-multisystem-disease","status":"publish","type":"post","link":"https:\/\/cov19longhaulfoundation.org\/?p=15780","title":{"rendered":"Mitochondrial and Metabolic Dysfunction in Long COVID: A Bioenergetic Model of Fatigue, Post-Exertional Malaise, and Multisystem Disease"},"content":{"rendered":"\n<h5 class=\"wp-block-heading\">Etiology, pathology, genomics, physiology, and clinical course<\/h5>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">Abstract<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Long COVID, or post-acute sequelae of SARS-CoV-2 infection (PASC), is a heterogeneous multisystem disorder in which fatigue, post-exertional malaise, cognitive dysfunction, autonomic disturbance, exercise intolerance, myalgia and cardiopulmonary symptoms frequently coexist. Although no single mechanism explains the syndrome, accumulating evidence implicates persistent disturbances of cellular energy metabolism. Mitochondria occupy a central position in this hypothesis because they integrate oxidative phosphorylation, substrate utilization, redox homeostasis, calcium signaling, apoptosis, innate immunity and cellular stress responses.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A growing body of physiological, transcriptomic, proteomic and metabolomic evidence indicates that some patients with Long COVID exhibit impaired oxidative phosphorylation, altered skeletal-muscle bioenergetics, increased reliance on glycolysis, abnormalities of tricarboxylic-acid-cycle metabolism, oxidative stress, altered amino-acid and lipid metabolism, and persistent activation of mitochondrial stress pathways. A 2026 multi-omic analysis integrating human and animal datasets reported sustained suppression of oxidative-phosphorylation programs and mitochondrial stress signatures extending into the post-acute period, with particularly prominent abnormalities in skeletal muscle.[1] A contemporaneous <em>Nature Communications<\/em> study provides important physiological evidence that skeletal-muscle abnormalities in Long COVID cannot be explained simply by physical deconditioning: muscle phenotypes and exercise responses differed from those produced by prolonged bed rest.[2]<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The mitochondrial hypothesis is nevertheless more nuanced than the proposition that SARS-CoV-2 simply &#8220;damages mitochondria&#8221;. Mitochondrial dysfunction may arise secondarily from persistent inflammation, immune-cell metabolic reprogramming, endothelial dysfunction, impaired oxygen delivery, viral proteins or nucleic acids, altered redox state, autoimmunity, autonomic disturbance, nutritional or metabolic stress, and persistent viral reservoirs. Mitochondrial impairment may itself then become a feed-forward mechanism: defective oxidative phosphorylation increases reactive oxygen species and cellular stress, promotes compensatory glycolysis, alters innate immune signaling, and reduces energetic reserve. In skeletal muscle, this may contribute directly to reduced exercise capacity and post-exertional malaise; in the brain, it may impair cognitive and autonomic function; in the heart, it may reduce energetic reserve; and in immune cells, it may perpetuate abnormal inflammatory states.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The genomic basis remains incompletely defined. Because oxidative phosphorylation depends upon coordinated expression of the nuclear and mitochondrial genomes, Long COVID may involve disturbances of mitochondrial gene expression, mitochondrial quality control, mitophagy, nuclear-encoded respiratory-chain proteins, mitochondrial dynamics and epigenetic regulation rather than conventional single-gene mitochondrial disease. Host genetic susceptibility may influence the ability to resolve inflammation and restore metabolic homeostasis. The clinical implication is that mitochondrial dysfunction should be regarded as a candidate <strong>mechanistic endotype<\/strong>, rather than a universal explanation for Long COVID. Definitive progress will require longitudinal integration of genomic, metabolomic, proteomic, physiological and tissue-level measurements, together with intervention studies capable of demonstrating that correction of the metabolic abnormality improves clinical outcomes.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">Introduction<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Fatigue is among the most characteristic and disabling manifestations of Long COVID.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Yet &#8220;fatigue&#8221; is an imprecise clinical term.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It can denote:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>sleepiness;<\/li>\n\n\n\n<li>generalized weakness;<\/li>\n\n\n\n<li>impaired motivation;<\/li>\n\n\n\n<li>cognitive exhaustion;<\/li>\n\n\n\n<li>reduced exercise capacity;<\/li>\n\n\n\n<li>muscular fatigue;<\/li>\n\n\n\n<li>autonomic intolerance;<\/li>\n\n\n\n<li>or the distinctive delayed deterioration known as post-exertional malaise (PEM).<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">In Long COVID, several of these phenomena frequently coexist.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The central physiological question is therefore not simply why patients <em>feel tired<\/em>, but why apparently modest physical or cognitive demands can produce disproportionate and sometimes delayed deterioration.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">One compelling explanation is a failure of <strong>bioenergetic reserve<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Every cellular function ultimately depends upon energy availability. Mitochondria generate most cellular ATP through oxidative phosphorylation (OXPHOS), while simultaneously regulating redox balance, calcium signaling, apoptosis, inflammatory pathways and cellular adaptation to stress.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Tissues with exceptionally high energetic requirements\u2014including skeletal muscle, myocardium, brain and immune cells\u2014should therefore be particularly vulnerable to persistent mitochondrial dysfunction.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This proposition has moved beyond theoretical speculation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A 2025 systematic review of omics studies in Long COVID identified mitochondrial dysfunction among recurring biological findings across symptom phenotypes, while emphasising substantial heterogeneity among studies.[3] A 2026 multi-omic analysis subsequently reported persistent suppression of OXPHOS and mitochondrial stress signatures across several tissues and biological compartments.[1] And physiological studies of skeletal muscle indicate abnormalities that cannot be explained simply by inactivity or deconditioning.[2]<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Taken together, these findings provide the basis for a coherent <strong>immunometabolic model of Long COVID<\/strong>.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">I. Mitochondria as the Cellular Integrator<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Mitochondria are traditionally described as the organelles responsible for ATP production.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That description is incomplete.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Mitochondria participate in:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>oxidative phosphorylation;<\/li>\n\n\n\n<li>fatty-acid \u03b2-oxidation;<\/li>\n\n\n\n<li>the tricarboxylic-acid (TCA) cycle;<\/li>\n\n\n\n<li>amino-acid metabolism;<\/li>\n\n\n\n<li>calcium buffering;<\/li>\n\n\n\n<li>reactive-oxygen-species signaling;<\/li>\n\n\n\n<li>apoptosis;<\/li>\n\n\n\n<li>innate immune signaling;<\/li>\n\n\n\n<li>cellular stress responses;<\/li>\n\n\n\n<li>steroid and haem synthesis;<\/li>\n\n\n\n<li>metabolic adaptation.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The respiratory chain consists of complexes I\u2013IV, which transfer electrons from reduced substrates to molecular oxygen while generating a proton gradient across the inner mitochondrial membrane. Complex V, ATP synthase, then uses that electrochemical gradient to generate ATP.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The process can be simplified as:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>substrates \u2192 NADH\/FADH\u2082 \u2192 electron transport \u2192 proton gradient \u2192 ATP synthesis.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A defect anywhere along this chain can reduce energetic efficiency.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But the physiological consequences are not restricted to ATP deficiency.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Electron leakage can increase mitochondrial reactive oxygen species (ROS), which can alter proteins, lipids, DNA and signaling pathways.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Mitochondria consequently represent both:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>energy-producing organelles<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">and<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>stress-sensing organelles<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This dual role makes them particularly important in Long COVID.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">II. The Bioenergetic Hypothesis<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">The simplest mitochondrial model proposes:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>SARS-CoV-2 infection<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>mitochondrial stress<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>impaired oxidative phosphorylation<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>reduced ATP availability<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>increased glycolytic compensation<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>metabolic inflexibility<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>exercise intolerance and fatigue.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The actual biology is more complicated.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The evidence increasingly suggests a dynamic network involving:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>inflammation \u2194 mitochondrial dysfunction \u2194 oxidative stress \u2194 metabolic reprogramming \u2194 immune activation.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Each component can reinforce the others.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">III. SARS-CoV-2 and Mitochondrial Biology<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">SARS-CoV-2 infection profoundly alters host-cell metabolism.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Viruses depend upon host cells for:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>nucleotides;<\/li>\n\n\n\n<li>amino acids;<\/li>\n\n\n\n<li>lipids;<\/li>\n\n\n\n<li>ATP;<\/li>\n\n\n\n<li>protein synthesis.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Viral infection therefore necessarily modifies cellular metabolism.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">During acute infection, host cells can undergo metabolic reprogramming toward glycolysis, altered lipid synthesis and modified mitochondrial signaling.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Several SARS-CoV-2 proteins have been reported experimentally to interact with mitochondrial or mitochondrial-associated processes, including pathways governing:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>mitochondrial membrane potential;<\/li>\n\n\n\n<li>innate immune signaling;<\/li>\n\n\n\n<li>ROS;<\/li>\n\n\n\n<li>apoptosis;<\/li>\n\n\n\n<li>mitochondrial dynamics.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The key question for Long COVID is whether these acute perturbations are <strong>completely reversed<\/strong> after viral clearance.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Increasing evidence suggests that in at least some patients they are not.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">IV. Persistent Mitochondrial Suppression<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">The 2026 multi-omic study by Tasoula and colleagues provides important recent evidence.[1]<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The investigators integrated transcriptomic, proteomic and metabolomic datasets from human cohorts and SARS-CoV-2-infected hamsters.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Across multiple tissues they identified recurring signatures of:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>suppressed OXPHOS;<\/li>\n\n\n\n<li>mitochondrial stress;<\/li>\n\n\n\n<li>inflammatory signaling;<\/li>\n\n\n\n<li>altered metabolism.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Skeletal muscle was particularly striking.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In experimental animals, OXPHOS-related transcriptional suppression persisted well beyond the acute phase. Human skeletal-muscle datasets from patients with post-COVID chronic fatigue demonstrated related mitochondrial and muscle-function abnormalities.[1]<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The significance lies in the convergence of independent biological measurements.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The hypothesis is no longer based solely on a single metabolite or enzyme.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Instead:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>transcriptomics + proteomics + metabolomics + tissue physiology<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">are beginning to point in the same direction.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Nevertheless, heterogeneity among cohorts remains considerable, and the findings should not yet be interpreted as proof that mitochondrial dysfunction is present in every patient with Long COVID.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">V. Skeletal Muscle as a Metabolic Organ<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Skeletal muscle is one of the body&#8217;s largest metabolic tissues.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">During exercise, ATP consumption rises dramatically.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">ATP must be continuously regenerated through:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>phosphocreatine buffering;<\/li>\n\n\n\n<li>glycolysis;<\/li>\n\n\n\n<li>oxidative phosphorylation;<\/li>\n\n\n\n<li>fatty-acid oxidation.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">At low workloads, these systems can compensate for one another.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">At high or prolonged workloads, oxidative phosphorylation becomes increasingly important.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Consequently, a defect in mitochondrial oxidative capacity may be largely invisible at rest but become clinically apparent during exercise.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This provides a plausible explanation for an important characteristic of Long COVID:<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><strong>Some patients can appear relatively well at rest yet become profoundly impaired after modest exertion.<\/strong><\/p>\n<\/blockquote>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">VI. Exercise Physiology<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Cardiopulmonary exercise testing (CPET) provides a window into this physiology.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Peak oxygen consumption (VO\u2082peak) reflects the integrated ability of the organism to:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>deliver oxygen;<\/li>\n\n\n\n<li>transport oxygen;<\/li>\n\n\n\n<li>extract oxygen;<\/li>\n\n\n\n<li>utilise oxygen within mitochondria.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">The Fick principle can be expressed as:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>VO\u2082 = cardiac output \u00d7 arterial\u2013venous oxygen difference.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Thus, a reduced VO\u2082peak does not automatically establish mitochondrial disease.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Possible causes include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>cardiac dysfunction;<\/li>\n\n\n\n<li>pulmonary limitation;<\/li>\n\n\n\n<li>anaemia;<\/li>\n\n\n\n<li>autonomic dysfunction;<\/li>\n\n\n\n<li>impaired peripheral oxygen extraction;<\/li>\n\n\n\n<li>deconditioning;<\/li>\n\n\n\n<li>mitochondrial dysfunction.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The challenge is to determine which component is limiting performance in an individual patient.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">VII. Long COVID Is Not Simply Deconditioning<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">This distinction is crucial.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients with chronic illness often become physically inactive.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Physical inactivity itself causes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>muscle atrophy;<\/li>\n\n\n\n<li>reduced mitochondrial capacity;<\/li>\n\n\n\n<li>reduced aerobic fitness;<\/li>\n\n\n\n<li>altered fibre composition.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">One might therefore argue that Long-COVID muscle abnormalities are simply consequences of inactivity.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Recent evidence argues against that being the complete explanation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A 2026 <em>Nature Communications<\/em> study compared Long-COVID and ME\/CFS patients with healthy individuals subjected to 60 days of strict bed rest.[2]<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Bed rest produced predictable physiological adaptations, but the skeletal-muscle and exercise responses of Long-COVID patients differed from those induced by bed rest.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The investigators concluded that physical inactivity alone could not account for the reduced exercise capacity and muscle adaptations observed in Long COVID.[2]<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This does not mean deconditioning is irrelevant.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Rather:<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><strong>Deconditioning may contribute to Long-COVID exercise limitation without being sufficient to explain it.<\/strong><\/p>\n<\/blockquote>\n\n\n\n<p class=\"wp-block-paragraph\">That distinction is clinically important.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">VIII. Skeletal-Muscle Fibre Remodeling<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Muscle fibres differ metabolically.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Type I fibres are highly oxidative and resistant to fatigue.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Type II fibres are more glycolytic and suited to rapid force generation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Recent skeletal-muscle studies have identified altered fibre characteristics in Long COVID, including a shift toward more glycolytic fibres and abnormalities in muscle metabolism.[2,4]<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This provides a potential physiological bridge between molecular findings and symptoms.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If muscle becomes relatively more dependent on glycolysis:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>ATP can still be generated rapidly<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">but<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>oxidative endurance may decline.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The patient may therefore tolerate brief activity while becoming markedly impaired by sustained activity.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">IX. What Would Prove the Mitochondrial Hypothesis?<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">A convincing causal demonstration would require several observations.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">First<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Mitochondrial abnormalities should precede or closely track symptom development.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">Second<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">The magnitude of mitochondrial impairment should correlate with physiological disability.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">Third<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">The abnormalities should be tissue-specific in biologically plausible ways.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">Fourth<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">They should distinguish Long COVID from ordinary deconditioning.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">Fifth<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Correcting the mitochondrial abnormality should improve physiological function.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">Sixth<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Clinical improvement should accompany restoration of metabolic homeostasis.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The final criterion is crucial.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A biomarker that merely accompanies disease is not necessarily a therapeutic target.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">XL. Research Priorities<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Future Long-COVID cohorts should integrate:<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">Genomics<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>whole-genome sequencing;<\/li>\n\n\n\n<li>mtDNA sequencing;<\/li>\n\n\n\n<li>HLA typing;<\/li>\n\n\n\n<li>epigenomics.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Transcriptomics<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>nuclear OXPHOS genes;<\/li>\n\n\n\n<li>mitochondrial transcripts;<\/li>\n\n\n\n<li>inflammatory pathways;<\/li>\n\n\n\n<li>metabolic regulators.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Proteomics<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>respiratory-chain proteins;<\/li>\n\n\n\n<li>mitochondrial stress proteins;<\/li>\n\n\n\n<li>inflammatory mediators.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Metabolomics<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>glycolysis;<\/li>\n\n\n\n<li>TCA cycle;<\/li>\n\n\n\n<li>fatty-acid oxidation;<\/li>\n\n\n\n<li>amino-acid metabolism;<\/li>\n\n\n\n<li>NAD\u207a pathways;<\/li>\n\n\n\n<li>redox metabolites.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Physiology<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>CPET;<\/li>\n\n\n\n<li>muscle spectroscopy;<\/li>\n\n\n\n<li>oxygen extraction;<\/li>\n\n\n\n<li>endothelial function;<\/li>\n\n\n\n<li>autonomic testing.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Tissue studies<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>skeletal-muscle biopsy when clinically justified;<\/li>\n\n\n\n<li>mitochondrial ultrastructure;<\/li>\n\n\n\n<li>respiratory-chain activity;<\/li>\n\n\n\n<li>mitochondrial DNA integrity.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Such integration could finally determine whether mitochondrial dysfunction represents:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>cause \u2192 consequence \u2192 amplifier \u2192 biomarker<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">or some combination thereof.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">XLI. Conclusions<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">The mitochondrial hypothesis has emerged as one of the most compelling explanations for the profound fatigue and exercise intolerance experienced by many patients with Long COVID.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The evidence, however, supports a more sophisticated proposition than the simple assertion that SARS-CoV-2 &#8220;damages mitochondria&#8221;.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The emerging model is one of <strong>persistent immunometabolic dysregulation<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Acute SARS-CoV-2 infection may initiate mitochondrial stress. In susceptible individuals, inflammatory signaling, oxidative stress, persistent antigen, viral reactivation, endothelial dysfunction, autonomic disturbance and metabolic reprogramming may prevent complete restoration of mitochondrial homeostasis.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The consequence is a reduction in <strong>bioenergetic reserve<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">At rest, this reduction may remain clinically subtle.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Under exertion, however, the deficit becomes apparent.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The patient crosses the threshold at which metabolic demand exceeds physiological reserve.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The result is exercise intolerance.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And in some patients, the subsequent inflammatory, autonomic and metabolic cascade produces delayed post-exertional malaise.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The recent evidence is particularly persuasive because the mitochondrial hypothesis is increasingly supported at several biological levels simultaneously. Multi-omic studies demonstrate persistent suppression of OXPHOS and mitochondrial stress pathways; metabolomic investigations identify altered substrate utilisation; skeletal-muscle studies demonstrate metabolic and structural abnormalities; and physiological experiments indicate that these abnormalities cannot be explained simply by inactivity.[1\u20133]<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Yet important uncertainties remain.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Mitochondrial dysfunction is unlikely to be universal.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It may represent one of several Long-COVID endotypes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Some patients may have predominantly vascular disease; others autonomic dysfunction, persistent viral antigen, immune dysregulation, autoimmunity or combinations of these mechanisms.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The most productive conceptual model is therefore not:<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><strong>Long COVID is a mitochondrial disease.<\/strong><\/p>\n<\/blockquote>\n\n\n\n<p class=\"wp-block-paragraph\">It is:<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><strong>In a subset of patients, SARS-CoV-2 initiates a persistent immunometabolic state in which impaired mitochondrial function reduces bioenergetic reserve and contributes to fatigue, post-exertional malaise and multisystem physiological dysfunction.<\/strong><\/p>\n<\/blockquote>\n\n\n\n<p class=\"wp-block-paragraph\">This hypothesis is biologically coherent, experimentally testable and increasingly supported by convergent evidence.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The decisive next step is not another descriptive metabolomic study.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is intervention.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If restoration of mitochondrial function produces restoration of physiological capacity and relief of symptoms, the mitochondrial hypothesis will move from association toward causality.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Until then, mitochondrial dysfunction should be regarded as a <strong>major candidate mechanism and potentially important therapeutic endotype of Long COVID, rather than a universal explanation for the disease<\/strong>.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">Numbered References<\/h5>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Tasoula A, Arif S, Waisberg E, et al.
