Cognitive dysfunction, fatigue, headache, sleep disturbance, dysautonomia, anosmia, neuropathic pain, sensory abnormalities, movement disorders, vestibular symptoms and neuropsychiatric disturbances may persist for months or years after the acute infection. The central biological question is no longer whether SARS-CoV-2 can produce persistent neurological disease, but how a transient systemic viral infection can generate prolonged dysfunction in neural, vascular, immune, autonomic and peripheral nervous systems.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Evidence accumulated through 2024\u201326 indicates that neurological Long COVID is unlikely to represent a single neuropathological entity. Instead, it comprises overlapping biological phenotypes involving persistent systemic inflammation, blood\u2013brain-barrier (BBB) dysfunction, endothelial injury, altered cerebral perfusion, neuroimmune activation, autonomic dysregulation, small-fiber neuropathy, metabolic disturbance and, in a subset of patients, persistent viral or antigenic stimulation. Dynamic-contrast MRI studies have demonstrated persistent BBB disruption in patients with cognitive Long COVID, accompanied by abnormalities of coagulation and adaptive immunity.\u00b9 Subsequent MRI work has reported subtle BBB permeability abnormalities more than two years after infection.\u00b2 Multimodal imaging has additionally identified alterations in white-matter microstructure, cerebral blood flow, brain metabolites and regional brain volume.\u00b3\u2013\u2076<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The neurological phenotype is also biologically heterogeneous. Some patients exhibit predominantly cognitive dysfunction; others have autonomic or small-fiber neuropathic disease, while still others develop headache, anosmia, vestibular symptoms, sleep disturbance or multisystem neurological illness. Recent histological studies provide evidence of autonomic small-fiber injury even in patients whose conventional sensory nerve-fiber density remains preserved.\u2077\u02d2\u2078<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Genomic evidence indicates that susceptibility to Long COVID is partly heritable. A 2025 genome-wide association study involving up to 6,450 Long-COVID cases and more than 1 million controls identified a significant association near <strong>FOXP4<\/strong>, although this locus appears to have a stronger relationship to pulmonary and systemic Long-COVID susceptibility than to a specifically neurological phenotype.\u2079 The neurological phenotype is therefore more appropriately understood as the result of interactions among host genetic susceptibility, viral characteristics, immune response, vascular biology, autonomic regulation and tissue-specific vulnerability.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The emerging model is consequently one of <strong>postinfectious neurovascular\u2013immune dysregulation rather than persistent primary viral encephalitis<\/strong>. Direct CNS infection may occur in selected circumstances, but available evidence increasingly supports indirect mechanisms\u2014particularly BBB dysfunction, endothelial activation, persistent immune signaling, altered neurovascular coupling and peripheral neuroimmune injury\u2014as major contributors to chronic neurological disease.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">Introduction<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">The neurological consequences of SARS-CoV-2 infection were recognized during the earliest months of the pandemic.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Initially, attention focused on acute neurological complications:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>encephalitis;<\/li>\n\n\n\n<li>stroke;<\/li>\n\n\n\n<li>seizures;<\/li>\n\n\n\n<li>anosmia;<\/li>\n\n\n\n<li>encephalopathy;<\/li>\n\n\n\n<li>Guillain\u2013Barr\u00e9 syndrome;<\/li>\n\n\n\n<li>cerebrovascular thrombosis.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The subsequent recognition of persistent neurological symptoms fundamentally changed the clinical landscape.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients who had survived the acute infection began reporting:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>inability to concentrate;<\/li>\n\n\n\n<li>impaired short-term memory;<\/li>\n\n\n\n<li>slowed thinking;<\/li>\n\n\n\n<li>word-finding difficulty;<\/li>\n\n\n\n<li>headache;<\/li>\n\n\n\n<li>dizziness;<\/li>\n\n\n\n<li>tinnitus;<\/li>\n\n\n\n<li>altered smell and taste;<\/li>\n\n\n\n<li>neuropathic pain;<\/li>\n\n\n\n<li>numbness;<\/li>\n\n\n\n<li>weakness;<\/li>\n\n\n\n<li>sleep disruption;<\/li>\n\n\n\n<li>orthostatic intolerance;<\/li>\n\n\n\n<li>tachycardia;<\/li>\n\n\n\n<li>exercise intolerance;<\/li>\n\n\n\n<li>post-exertional deterioration.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The term <strong>brain fog<\/strong> became the common clinical description.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Yet brain fog is not a diagnosis.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is a phenotype.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This distinction is essential because the neurological manifestations of Long COVID are increasingly understood to represent several interacting biological processes rather than a single pathological lesion.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The contemporary neurological model can therefore be expressed as:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>SARS-CoV-2 infection<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>systemic and tissue-specific immune response<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>vascular, endothelial and autonomic disturbance<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>BBB dysfunction \u00b1 peripheral nerve injury \u00b1 neuroimmune activation<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>altered cerebral perfusion and neural network function<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>cognitive, sensory, autonomic and neurological symptoms.<\/strong><\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">1. The Neurological Phenotype<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Neurological Long COVID encompasses at least seven partially overlapping domains.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">Cognitive<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>impaired attention;<\/li>\n\n\n\n<li>executive dysfunction;<\/li>\n\n\n\n<li>working-memory impairment;<\/li>\n\n\n\n<li>slowed processing speed;<\/li>\n\n\n\n<li>word-retrieval difficulty;<\/li>\n\n\n\n<li>impaired learning.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Autonomic<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>postural orthostatic tachycardia syndrome;<\/li>\n\n\n\n<li>orthostatic intolerance;<\/li>\n\n\n\n<li>inappropriate sinus tachycardia;<\/li>\n\n\n\n<li>abnormal sweating;<\/li>\n\n\n\n<li>gastrointestinal dysmotility;<\/li>\n\n\n\n<li>thermoregulatory abnormalities.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Peripheral neuropathic<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>burning pain;<\/li>\n\n\n\n<li>paresthesias;<\/li>\n\n\n\n<li>numbness;<\/li>\n\n\n\n<li>altered temperature sensation;<\/li>\n\n\n\n<li>dysesthesia;<\/li>\n\n\n\n<li>small-fiber neuropathy.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Cerebrovascular\/neurovascular<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>headache;<\/li>\n\n\n\n<li>dizziness;<\/li>\n\n\n\n<li>cerebral hypoperfusion;<\/li>\n\n\n\n<li>impaired neurovascular coupling.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Sensory<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>anosmia;<\/li>\n\n\n\n<li>dysgeusia;<\/li>\n\n\n\n<li>tinnitus;<\/li>\n\n\n\n<li>visual disturbances;<\/li>\n\n\n\n<li>vestibular symptoms.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Sleep and neuropsychiatric<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>insomnia;<\/li>\n\n\n\n<li>hypersomnia;<\/li>\n\n\n\n<li>fragmented sleep;<\/li>\n\n\n\n<li>anxiety;<\/li>\n\n\n\n<li>depression;<\/li>\n\n\n\n<li>altered arousal.<\/li>\n<\/ul>\n\n\n\n<h5 class=\"wp-block-heading\">Motor<\/h5>\n\n\n\n<ul class=\"wp-block-list\">\n<li>weakness;<\/li>\n\n\n\n<li>impaired coordination;<\/li>\n\n\n\n<li>tremor;<\/li>\n\n\n\n<li>exercise intolerance.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The phenotypes frequently coexist.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A patient with small-fiber neuropathy may simultaneously have POTS, fatigue and cognitive dysfunction. Thus, neurological Long COVID is better represented as a <strong>network of interacting phenotypes<\/strong> than as discrete diseases.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">2. Etiology: From Viral Infection to Chronic Neurological Dysfunction<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">The initiating event is SARS-CoV-2 infection.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The unresolved question is why neurological symptoms persist after the acute infection has apparently resolved.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Several non-mutually exclusive mechanisms have emerged.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">2.1 Persistent viral antigen<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">SARS-CoV-2 RNA or protein has been detected in tissues after acute infection in some studies.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The critical distinction is between:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>persistent replication<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">and<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>persistent antigenic material<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The latter could continue stimulating immune pathways without requiring high-level productive viral replication.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This mechanism could produce a chronic state of:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>antigen \u2192 innate immune activation \u2192 endothelial dysfunction \u2192 neuroimmune signaling.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Evidence for persistent viral material does not establish that it is responsible for every neurological phenotype.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">2.2 Persistent systemic inflammation<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Patients with neurological Long COVID may exhibit persistent abnormalities in immune signaling.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A large-scale mechanistic phenotyping study found persistent myeloid inflammation and complement activation across several Long-COVID clinical phenotypes, including cognitive and fatigue groups.\u00b9\u2070<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is significant because complement and myeloid pathways can affect:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>endothelial permeability;<\/li>\n\n\n\n<li>platelet activation;<\/li>\n\n\n\n<li>leukocyte trafficking;<\/li>\n\n\n\n<li>synaptic homeostasis;<\/li>\n\n\n\n<li>microglial activation.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The brain therefore need not be directly infected for neurological dysfunction to occur.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">3. The Blood\u2013Brain Barrier as a Central Pathological Interface<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">The BBB is one of the most compelling biological bridges between systemic COVID-19 and chronic neurological disease.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The barrier consists principally of:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>endothelial cells;<\/li>\n\n\n\n<li>tight junctions;<\/li>\n\n\n\n<li>pericytes;<\/li>\n\n\n\n<li>basement membrane;<\/li>\n\n\n\n<li>astrocytic endfeet.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Its physiological function is to regulate movement of molecules and immune cells between blood and neural tissue.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">SARS-CoV-2-associated systemic inflammation can disrupt this equilibrium.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The resulting sequence may be:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>endothelial activation<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>tight-junction alteration<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>increased permeability<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>entry of inflammatory mediators<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>microglial\/astrocytic activation<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u2193<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>altered synaptic and neuronal function.<\/strong><\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">4. Direct Evidence for BBB Dysfunction<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Greene and colleagues provided some of the strongest evidence for BBB involvement in cognitive Long COVID.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Using dynamic contrast-enhanced MRI, they identified increased BBB permeability in patients with persistent brain fog. Peripheral blood mononuclear cells exhibited increased adhesion to human brain endothelial cells, while serum from affected patients induced inflammatory responses in brain endothelial cells in vitro.\u00b9<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The importance of this study is conceptual.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It connects:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>systemic immune abnormalities<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">with<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>brain vascular dysfunction<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">and<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>cognitive symptoms<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A 2025 MRI study involving 97 individuals with Long COVID and neuropsychiatric symptoms likewise reported increased BBB permeability compared with recovered controls, although cerebral blood flow and global brain volume did not differ significantly.\u00b2<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Thus, BBB abnormalities may persist even when conventional structural MRI appears relatively normal.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">5. White-Matter Pathology<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">The white matter is particularly vulnerable to vascular and inflammatory perturbations.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Diffusion MRI provides indirect measures of white-matter microstructure.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A multimodal longitudinal study of patients with post-COVID cognitive impairment found abnormalities involving:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>BBB permeability;<\/li>\n\n\n\n<li>white-matter integrity;<\/li>\n\n\n\n<li>brain metabolites.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Some white-matter abnormalities improved during follow-up, whereas others persisted.\u00b3<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">More recent diffusion MRI work in individuals with neurological PASC persisting for approximately 2.7 years demonstrated global and tract-specific white-matter abnormalities, including prominent changes involving the fornix and minor forceps.\u2074<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">These findings are intriguing because the affected pathways participate in:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>memory;<\/li>\n\n\n\n<li>executive processing;<\/li>\n\n\n\n<li>limbic integration.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">However, diffusion abnormalities cannot by themselves distinguish:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>inflammation;<\/li>\n\n\n\n<li>demyelination;<\/li>\n\n\n\n<li>axonal injury;<\/li>\n\n\n\n<li>altered extracellular water.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Consequently, they should be interpreted as evidence of altered tissue microstructure rather than proof of a particular histopathological lesion.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">6. Neuroinflammation<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">Neuroinflammation is one of the dominant mechanistic hypotheses.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Potential cellular participants include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>microglia;<\/li>\n\n\n\n<li>astrocytes;<\/li>\n\n\n\n<li>endothelial cells;<\/li>\n\n\n\n<li>perivascular macrophages;<\/li>\n\n\n\n<li>infiltrating monocytes.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Microglia are the resident immune cells of the CNS.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Persistent activation can alter:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>synaptic pruning;<\/li>\n\n\n\n<li>neurotransmitter signaling;<\/li>\n\n\n\n<li>neuronal metabolism;<\/li>\n\n\n\n<li>network connectivity.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Astrocytes regulate:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>extracellular potassium;<\/li>\n\n\n\n<li>glutamate;<\/li>\n\n\n\n<li>metabolic support;<\/li>\n\n\n\n<li>BBB integrity;<\/li>\n\n\n\n<li>synaptic function.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Abnormal astrocytic signaling could therefore produce substantial neurological dysfunction without causing widespread neuronal death.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This distinction may explain why patients can have severe cognitive symptoms despite relatively modest conventional MRI abnormalities.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h5 class=\"wp-block-heading\">7.
